Home Health Visits with Growth Potential
Job Summary:
We are seeking a dedicated and skilled Occupational Therapist (OT) to join our team. This role focuses on providing home health services and includes opportunities to offer Remote Therapeutic Monitoring (RTM) and Outpatient Therapy. The ideal candidate will deliver patient-centered care while adhering to Medicare and Medicaid standards.
Key Responsibilities:
  • Conduct home-based occupational therapy assessments, admissions, and follow-up visits.
  • Develop and implement individualized therapy plans to improve daily living and occupational skills.
  • Provide RTM services, including device setup, monitoring, and therapy adjustments.
  • Educate patients and caregivers on therapy techniques and assistive devices.
  • Document patient progress and adhere to Medicare and Medicaid compliance guidelines.
