Intake Nurse - RN or LPN

Springdale, AR, USA

Full Time

This position will work 8:30-5 or 9-5:30, with weekend and holiday rotation.

The Intake Nurse is integral in the coordination and execution of the admission process to Hospice services. Demonstrates excellent customer service skills with patients, families, customers, and co-workers. Must be resourceful in overcoming hospice barriers with patients and caregivers. Responsible for obtaining referral orders and patient records.

Unrestricted LPN or RN license. Minimum of 2 years of nursing experience in home health, or healthcare facility. Must be able to deal with human suffering while maintaining professional boundaries. Required to be flexible and change schedule at a moment's notice to meet patient needs. Must have excellent clinical skills and be able to identify/solve problems, consult with the physician, and execute orders to meet patient needs.

  2. Ability to multi-task in a high stress environment with incoming phone calls, detailed data entry, compassionate and non-rushed communications with families, professional conversations with physicians, clinic and facility staff and hospitals.
  3. Communicates the Hospice philosophy to families and patients effectively, at their pace and with
  4. Facilitates the timely conversion of referrals to admissions through coordination with team members and other departments as needed.

  10. Coordinate scheduling of APRN or Physician face-to-face (F2F) appointments
  11. Follow up with referral sources, including clinics, SNF/ALF facilities, and Account Executives (AEs).
  12. Close pending lists in Referral Manager (RM) and Wellsky for patients who are no longer being followed or have
  13. Request medical records for home-based admissions from clinics, hospitals, and
  14. Obtain medical records as needed based on requests from admission nurses, liaisons, and Hospice
  15. Enter and document Care Navigation (Care Nav) referrals, including task notes, contact information, and service
  16. Enter Palliative Care referrals, track consult times, ensure consult notes are signed, email billing info, and monitor patient discharges.
  17. Support the entry and update of new referral accounts and physicians in Referral Manager (RM) for Arkansas and
  18. Support the entry and update new referral accounts and physicians in SmartMD for Arkansas and Oklahoma
  19. Provide phone support for patients, families, referral sources, admission nurses, and
  20. Assist with updating daily scheduled admissions and status
  21. Attention to timely determination of Attending MD for each admission, including detailed documentation of this
  22. May assist with Care Navigation calls to patients, in person for informational visits, and admissions as
  23. May be cross trained in Admissions and/or Liaison work, cover visits, and other areas when
  24. Assists with the development of care and effectively communicates the patient/family care needs to the
  25. Demonstrates compassion, empathy, and patience when interacting with patients, families, co-workers, and members of the public, in person, or on the phone.

  27. Demonstrates flexibility, versatility, and a positive attitude with integrating additional duties and flexible scheduling availability for weekend and after hours as needed.
