Overview:Join Iowa Home Care as a Registered Nurse Case Manager, where you’ll play a crucial role in providing compassionate, patient-centered care directly within clients' homes. Develop and update individualized care plans based on health assessments, including therapeutic, preventive, and rehabilitative actions.Direct Patient Care: Administer medications and treatments as prescribed, provide skilled nursing care per the State Nurse Practice Act, and implement preventive and rehabilitative procedures. Offer guidance to patients and families to help them understand and manage their health.Care Coordination: Coordinate care across services and ensure seamless support. Collaborate with physicians and community health providers to optimize care plans and outcomes.Documentation & Communication: Maintain accurate and timely clinical notes, updating the primary physician at least every 60 days or as needed. Communicate any changes in patient status to the physician and obtain necessary orders.Patient & Family Education: Provide health instructions tailored to the patient’s needs and care plan.
