Licensed Practical Nurse (LPN) – Home Health
Grants Pass, OR | Full-Time | Monday–Friday

Are you an experienced Licensed Practical Nurse (LPN) ready for a role where your nursing expertise directly impacts the lives of patients and their families?
We are currently seeking a compassionate and dependable LPN to provide skilled nursing care to patients throughout the Grants Pass area. This is a great opportunity for a nurse who enjoys building relationships with patients, working independently, and providing personalized care outside of a traditional clinical setting.
You'll be supported by an experienced healthcare team while having the independence to manage your visits and deliver care where patients need it most — in their own homes.
 Position Highlights

 Service Area
The position is based out of Grants Pass, Oregon, with patient visits throughout the surrounding communities:
Grants Pass • Hugo • Merlin • Rand • Sunny Valley • Gold Hill • Rogue River • Wimer • Cave Junction • Dryden • Kerby • O'Brien • Redwood • Williams • Wonder
A valid driver's license, reliable transportation, and the ability to travel throughout the service area are required.
 Your Role
As a Home Health LPN, you'll play an important role in helping patients manage their health and remain independent at home. Your responsibilities will include:

  • Conducting in-home nursing visits based on established care plans and physician orders.

  • Monitoring patient progress and identifying changes that may require additional attention.

  • Providing medications, treatments, and other prescribed nursing interventions.

  • Teaching patients and caregivers about medications, treatment plans, health conditions, and ongoing care.

  • Maintaining accurate and timely patient records and clinical documentation.

  • Communicating with physicians, therapists, case managers, and other healthcare professionals.

  • Helping patients achieve the best possible outcomes while maintaining their comfort, safety, and dignity.

  • Responding to patient needs as part of an established on-call rotation.


 What You'll Need

  • At least 1 year of clinical nursing experience.

  • At least 1 year of home health experience.

  • Graduate of an accredited practical nursing or vocational nursing program.

  • Active and unrestricted Oregon LPN license.

  • Current CPR certification.

  • Valid driver's license and reliable transportation.

  • Ability to travel between patient homes throughout the assigned territory.

  • Strong communication, organization, and clinical documentation skills.

  • Comfortable working independently and managing multiple patient needs.
