Speech Therapist - Medicare Home Health

PRN/Part Time โ€“ Home Health

Territory: Titusville, Melbourne, and Palm Bay (Brevard County)

Overview:
We are seeking a compassionate and skilled Speech Therapistย (ST) to provide high-quality, patient-centered speech therapy services on a PRN basis in the home health setting. In this role, you will work one-on-one with patients to address communication, swallowing, and cognitive-linguistic needsโ€”helping them regain function and improve their quality of life from the comfort of their homes.

This role offers flexible scheduling, independence in the field, and the support of a collaborative clinical team.


Key Responsibilities:
  • Conduct comprehensive speech, language, cognitive-linguistic, and swallowing evaluations.

  • Develop individualized treatment plans based on evaluation findings and patient goals.

  • Provide skilled therapy interventions, including communication therapy, swallowing therapy, cognitive retraining, and compensatory strategy training.

  • Educate patients and caregivers on home exercises, safety, and communication strategies.

  • Document all care in compliance with Medicare and agency standards.

  • Communicate regularly with nurses, therapists, and other interdisciplinary team members to ensure coordinated care.

  • Report any significant changes in patient condition to the physician and clinical team.

  • Active Florida Speech Therapist License.

  • Home health experience

  • Strong clinical assessment and communication skills.

  • Reliable transportation, valid driverโ€™s license, and current auto insurance.

  • Ability to work independently and manage a PRN caseload.
