Mobile Advanced Practice Provider (APRN / PA)  

Full-Time

Company: 5-50 Group

Location: South Florida (Miami-Dade, Broward, and Palm Beach Counties)

Mobile field role with remote documentation and care-team collaboration. Attendance at the Sunrise office as needed.

Reports To: Medical Director / Supervising Physician / Clinical Operations Leadership

Position Summary

5-50 Group is seeking a highly skilled Mobile Advanced Practice Provider (APRN or Physician Assistant) to deliver proactive, high-quality in-home and facility-based care for high-risk and medically complex patients.
This role is ideal for a clinically independent provider who is comfortable working in the field while leveraging remote patient monitoring, AI-enabled clinical workflows, and physician oversight to deliver coordinated, data-informed care.
The ideal candidate has strong experience in cardiology, pulmonology, urgent care, chronic disease management, post-acute care, geriatrics, or home-based medical services, and is highly proficient in clinical documentation and care coordination.
Employment Type

  • Full-time (preferred based on patient volume growth) 
  • Weekend coverage as needed 

Compensation

  • $112,000 – $140,000 annually, based on experience and qualifications 
  • Additional compensation structures may apply based on workload, productivity, and role classification (full-time vs. per-diem) 

Key Responsibilities

  • Conduct in-home and facility-based assessments for high-risk, post-acute, chronic care, workers’ compensation, and medically complex patients 
  • Visit skilled nursing facility (SNF) residents in person at least once per month for assigned patients 
  • Conduct home visits according to patient tier protocol, including: 
    • Weekly visits (high-risk tier) 
    • Every two weeks (moderate-risk tier) 
    • As-needed visits based on clinical status, escalations, or care coordination needs 
  • Obtain comprehensive histories, perform physical exams, review medications, and assess clinical risk 
  • Order, review, and interpret diagnostics (labs, imaging, EKGs, and other tests within scope/protocol) 
  • Develop, implement, and update patient care plans in coordination with physicians, specialists, and care teams 
  • Utilize remote monitoring data, clinical dashboards, and care manager escalations to guide timely interventions 
  • Respond to patient escalations and clinical calls per organizational protocols 
  • Document all clinical findings, decision-making, orders, communications, and follow-up plans accurately and promptly 
  • Collaborate with care managers and field staff to ensure continuity of care 
  • Support clinical education and guidance for field staff as needed 
  • Participate in quality improvement initiatives and AI-enabled workflow validation 
  • Contribute to refinement of clinical protocols and technology-driven care processes 

Required Qualifications

  • Active Florida ARNP/APRN or Physician Assistant (PA) license in good standing 
  • Minimum 2 years of clinical experience in one or more of the following areas: 
    • ICU / CCU 
    • Cardiology or Pulmonology 
    • Emergency Medicine or Urgent Care 
    • Hospitalist or Internal Medicine 
    • Geriatrics or complex chronic care 
    • Home-based or post-acute care 
  • Experience with high-acuity or medically complex patients strongly preferred 
  • Prior experience with home visits, telehealth, or remote patient monitoring strongly preferred 
  • Strong clinical judgment and ability to work independently in the field 
  • Excellent documentation and EMR proficiency 
  • Valid driver’s license, reliable transportation, and insurance 
  • Ability to travel throughout the assigned South Florida territory 
  • Authorized to work in the United States 
  • Professional English communication required 

Preferred Qualifications

  • Bilingual: Spanish strongly preferred; Haitian Creole is a plus 
  • Experience with: 
    • Cardiopulmonary assessment and monitoring 
    • EKG interpretation support 
    • Medication reconciliation and polypharmacy management 
    • IV skills (preferred, not required) 
  • Experience supervising or coordinating care teams, EMTs, MAs, or care managers 
  • Familiarity with digital health platforms, dashboards, or AI-supported clinical tools 
  • Interest in scalable, technology-enabled home healthcare models 

Technology & Workflow Expectations

  • Use StreamCare EMR, dashboards, and remote monitoring platforms for clinical decision-making 
  • Integrate patient-generated data and monitoring alerts into care planning 
  • Ensure documentation is clear, timely, and usable across multidisciplinary teams 
  • Validate and interpret AI-supported alerts using clinical judgment 
  • Identify workflow gaps and contribute to continuous improvement of care protocols 
  • Maintain compliance with physician-supervised practice standards at all times
