We are currently seeking Licensed Therapist - Intensive Home-Based Treatment (IHBT) to join our OhioRISE program. As a Licensed Therapist - IHBT, you'll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. Key Responsibilities
  • Adheres to the CareStar Rule in performance of job responsibilities.
  • Understands and complies with CareStar Policies and Procedures.
  • Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
  • Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
  • Conducts comprehensive assessments and develop individualized treatment plans.
  • Provides individual, family, and group therapy using evidence-based practices.
  • Delivers services 3-5 hours per week per client over a 3-6 month period.
  • Implements crisis intervention and safety planning as needed.
  • Coordinates referrals and link families to additional resources and supports.
  • Participates in multidisciplinary team meetings and case reviews.
  • Maintains accurate and timely clinical documentation in accordance with agency and Medicaid standards.
  • Tracks client progress and adjust treatment plans as needed.
  • Ensures compliance with HIPAA, licensing, and ethical standards.
  • Works closely with caregivers to support parenting strategies and family functioning.
  • Promotes culturally competent and trauma-informed care.
  • Empowers families to build natural supports and long-term stability.
  • Performs other projects and duties as assigned.

Minimum Qualifications
  • Master's degree in Social Work, Counseling, Psychology, or related field.
  • Independent licensure (LISW, LPCC, LMFT) or working toward independent licensure.
  • Experience working with youth and families in home or community-based settings.
  • Knowledge of Medicare, Medicaid, insurance and waiver programs and community resources.
  • Good problem-solving skills.
  • Effective verbal and written communication skills with attention to detail in assessment and in documentation.
  • Excellent organizational skills with the ability to prioritize projects and be attentive to detail.
  • Effective interpersonal skills with respect for others, including consumers, co-workers and other professionals within a diverse work environment.
  • Demonstrates strong critical thinking and problem solving skills and flexibility in relationship to job requirements.
  • Computer skills sufficient to enter and process consumer information, report preparation, and use CareStar's website and email, and familiarity with suite of Microsoft Office programs.
