Job description

Home-Based Primary Care Physician

This is an open continuous announcement and will remain open until 08/03/2026. The initial cut-off date for referral of eligible applications will be 06/15/2026, with subsequent cut-off dates every 2 weeks. The HBPC physician will be responsible for the overall medical care of patients with conditions requiring treatment in their specialty to include consultative, prescriptive and ordering authority in the outpatient/ambulatory and or inpatient basic setting at the VA Southern NV Health Care System. The incumbent provides patient care directly and/or through the clinical oversite of trainees to include residents, fellows and/or medical students in the home setting. In addition to their role as clinician, the HBPC physician will have administrative responsibilities for participation in HBPC development and programs, as well as an effective interface with the HBPC staff. The HBPC Physician will participate fully in all accreditation processes, reviews and site visits. This position encompasses the duties of clinical, administrative, teaching, research and other duties as assigned within the Geriatrics and Extended Care service line which includes a Home-Based Primary Care, Geriatric Evaluation Clinic, and Hospice and Palliative Care.

The major duties and responsibilities include, but are not limited to:

  • Participates fully in all accreditation processes, reviews and site visits.
  • Drives to patients homes. Position requires driving up to 25% of the time.
  • Provides direct patient in patients homes care which includes assessments, diagnosis, treatment and outcome measures of healthcare problems. This includes the following: initiating and adjusting therapeutic regimens, performing histories and physicals, ordering diagnostic and therapeutic tests/studies, initiating consults and referrals, and prescribing medications/supplies in accordance with VA policy, VA formulary, Federal & state regulations and scope of practice.
  • Collaborates with in house specialty clinics as well as community providers.
  • Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex patient care and clinical program management.
  • Facilitates academic activities to teach (didactics) and provide oversite for trainees including geriatric fellows, palliative medicine fellows, medical residents and trainees in other health-related disciplines.
  • Provides clinical input and oversight for treatment plans.
  • Is readily available to the team members for collaboration when medical or other problems arise.
  • Participates in and leads weekly HBPC team meetings.
  • Keeps the HBPC team apprised of medical care advances and practice standards.
  • May perform other duties as required/assigned.
  • Must have a valid drivers license.
