This role operates administratively in a Home and Community-Based Services (HCBS) Setting, defined as "medical, social, and supportive services for Iowans with functional, cognitive, and other physical or mental health needs" (Iowa Health & Human Services). Rather than institutionalizing individuals who have these physical and mental health needs, HCBS settings provide these individuals with the opportunity to live and receive services in a way honoring to them and their abilities.

  • Team Leadership & Culture Development: Build and maintain a strong team culture through active presence, monthly team meetings, staff coaching, on-call participation, and providing coverage as needed, including short-notice staffing gaps. 
  • Resource & Operational Oversight: Manage and monitor key resources including vehicle usage, individual budgets, payee-related financial processes, medication check-ins, and appointment scheduling to ensure services run smoothly and efficiently. 
  • Person-Centered Service Coordination: Ensure services are delivered in a way that respects individual needs and goals by collaborating with CBCM and IDT, developing individualized plans, completing assessments, and maintaining accurate service documentation and authorizations. 
  • Communication & Community Collaboration: Serve as the primary liaison for individuals served, families, guardians, and case managers; communicate service changes promptly, document interactions, and support community partnerships and service promotion efforts. 
  • Compliance, Documentation & Advocacy: Maintain compliance with all applicable laws, policies, and regulations; ensure accurate incident reporting and service documentation; and advocate for individuals to promote independence, dignity, and self-determination.
