Job description

Transform Your Nursing Career with Support Broker Services, LLC!

Are you a Registered Nurse ready to take your career to new heights, beyond the hospital environment? Join us in a rewarding role where your clinical skills create meaningful change and your empathy fosters life-changing connections. Support Broker Services, LLC is seeking dedicated RNs to embark on a fulfilling journey in personalized home health, providing exceptional one-on-one care to participants in Wisconsin's innovative IRIS (Include, Respect, I Self-Direct) program. This isn't just another nursing job—it's a transformative experience where your involvement becomes the foundation of someone's wellbeing.

What Sets This Role Apart:

Autonomy & Influence: Utilize your full nursing capabilities in a setting where your judgments directly affect patient comfort, safety, and overall quality of life.

Clinical Specialized Care: Become proficient in managing complex seizure conditions and offering behavioral support for individuals living with Lennox-Gastaut Syndrome (LGS) and treatment-resistant epilepsy.

Deep Connections: Build trusted relationships with your patient and their families, all in the familiar environment of their home—free from the fast-paced nature of institutional healthcare.

Flexible Scheduling: Create a work-life balance that suits you with options for 12-hour shifts—choose between day (7 AM – 7 PM) or night (7 PM – 7 AM) shifts.

About Us:

Support Broker Services is transforming the landscape of home healthcare with our personalized, participant-led support approach. Collaborating with Forward Health, we empower individuals with complex medical needs to live independently and fully.

Your Responsibilities:

Deliver skilled nursing care including advanced seizure management and proactive behavioral strategies. Collaborate with a dedicated multidisciplinary team to design and implement individualized care plans. Train and guide family members and support staff on best practices in care techniques. Advocate clinically within the client's home, ensuring every interaction is filled with dignity and respect.

Who We're Seeking:

A Wisconsin-licensed RN with a passion for purposeful and curious clinical practice. A nurse with expertise in neurology, epilepsy management, behavioral health, or complex home care settings. A compassionate critical thinker who flourishes in independent, fast-paced environments. An individual who meets all Wisconsin DHS caregiver standards, including background checks and registry requirements.

What We Provide:

Competitive Pay: Earn between $25.00 – $35.75/hour based on your experience and specialization.

Career Development: Step into a leadership role in the evolving field of home-based complex care.

Collaborative Support: Join a team that prioritizes clinical excellence, innovation, and compassion.

Personal Fulfillment: Experience the daily impact of your care in a tangible way.

Equal Employment Opportunity Statement:

At Support Broker Services, LLC, we celebrate diversity and are committed to fostering an inclusive workplace. We adhere to all Wisconsin employment regulations regarding discrimination.
