In this role you will:
Evaluate patient functional abilities and daily living skills
Develop individualized treatment plans to improve independence
Teach patients adaptive strategies and new ways to complete daily activities
Identify home safety risks and recommend modifications or equipment
Educate patients and caregivers on safe mobility and self-care
Collaborate with physicians, nurses, and therapy team members
Your work helps patientsremain safely in their homes and return to the activities that give life meaning.

What You'll Be Doing
Evaluating patient functional status and independence with daily activities
Assessing home environments for safety risks and barriers
Developing individualized treatment plans and therapy goals
Teaching adaptive techniques for daily living skills
Recommending and training patients on assistive equipment
Educating caregivers and family members
Supervising Certified Occupational Therapy Assistants when appropriate
Collaborating with the interdisciplinary home health team

What Helps You Succeed
Graduate of an Occupational Therapy program accredited by ACOTE
Eligible for or certified by NBCOT
Licensed Occupational Therapist in the State of Oregon
Strong communication and organizational skills
Current CPR certification
Valid driver's license and reliable transportation
Two years of clinical experience is preferred.
