Full-Time Hospice Chaplain Community Hospice | Albany/Rensselaer County Regions
Schedule: Monday through Friday, 8:00 a.m. to 4:30 p.m.
The Community Hospice has an exceptional opportunity for a compassionate and dedicated Full-Time Chaplain to join our Homecare Team serving Albany and Rensselaer Counties. As a valued member of our interdisciplinary hospice team, you will provide spiritual care, emotional support, and guidance to patients and families facing serious illness and end-of-life journeys.
This role is ideal for someone who enjoys meaningful patient interactions while collaborating with a highly supportive team of healthcare professionals, including RNs, LPNs, hospice aides, medical directors, social workers, complementary therapists, chaplains, and volunteers.
Position Summary
The Hospice Chaplain serves as a key member of the interdisciplinary care team and is responsible for coordinating spiritual care services for patients and families of all faith traditions and backgrounds. The Chaplain provides spiritual counseling, emotional support, and bereavement-related guidance while contributing to the development and implementation of individualized patient and family care plans. In this role, you will have the opportunity to make a meaningful difference by helping patients and their loved ones find comfort, peace, and support during life's most challenging moments.
What You'll Do
  • Provide spiritual counseling and emotional support to hospice patients, families, caregivers, and team members.
  • Assess spiritual needs and develop appropriate plans of care in collaboration with the interdisciplinary team.
  • Participate in care planning meetings and contribute to holistic patient care.
  • Support patients and families of diverse faith traditions, beliefs, and cultural backgrounds.
  • Coordinate spiritual care services and community faith-based resources as needed.
  • Conduct memorial services and provide bereavement support when appropriate.
  • Represent Community Hospice through community outreach and public speaking engagements.

What You'll Need
  • Master's degree from an accredited institution in Theology, Pastoral Care, Spirituality, or a related field required.
  • One to three years of chaplaincy or pastoral care experience, preferably in a healthcare setting.
  • Hospice experience and/or formal education in pastoral ministry preferred.
  • Active participation and good standing within your faith tradition, with documentation available upon request.
  • Strong interpersonal and communication skills, with sensitivity to family dynamics and end-of-life issues.
  • Ability to work effectively and collaboratively within an interdisciplinary team environment.
  • Ability and willingness to facilitate memorial services, conduct public speaking engagements, and occasionally participate in evening and/or weekend events.

Pay Range: $26.00 - $33.60
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
