Key Responsibilities

Your role will entail a variety of responsibilities that are essential to delivering holistic care:

  • Collaborate with primary care providers to identify, assess, and manage mental health and psychosocial challenges faced by patients.
  • Conduct patient intakes focused on diagnostic evaluations and make informed recommendations for treatment plans to primary care providers.
  • Provide brief and focused therapeutic interventions for clients needing mental health assistance, establishing clear treatment goals and strategies.
  • Monitor patient progress and modify treatment approaches based on ongoing assessments and feedback, including initiating follow-up to track developments.
  • Design individualized relapse prevention plans to help patients maintain stability in their mental health.
  • Detect at-risk patients and implement strategies to prevent the deterioration of their conditions.
  • Document patient progress meticulously in treatment charts while keeping primary care providers informed of changes and needs.
  • Facilitate linkages to external treatment resources in partnership with other providers, ensuring comprehensive support for your patients.
  • Provide educational resources and self-management strategies that enable patients to be active participants in their treatment.
  • Consult with clinic management and team members regarding behavioral health services and programs for outcome evaluations.
  • Assist patients in adhering to primary care treatments, including managing medication side effects.

Compensation and Benefits
