Role Description This full-time, on-site role in Bridgeport, CT is for Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), Home Health Aides (HHAs), Homemakers, and Companions. Team members will provide direct care and support in clients’ homes, including assistance with activities of daily living, basic medical tasks as allowed by licensure, monitoring client conditions, and reporting changes to supervisors. Responsibilities may include personal care, mobility support, meal preparation, light housekeeping, medication reminders, and companionship to enhance clients’ quality of life. RNs and LPNs may perform nursing assessments, care planning, wound care, and coordination with physicians, while CNAs, HHAs, Homemakers, and Companions focus on hands-on daily support. All staff are expected to follow care plans, maintain accurate documentation, uphold safety and infection control standards, and communicate respectfully with clients, families, and the care team.
Qualifications
  • Current, valid licensure or certification as RN, LPN, CNA, or HHA in the state of Connecticut, as applicable to the role.
  • Experience providing direct patient or client care in home care, long-term care, or similar settings.
  • Strong skills in personal care, assistance with activities of daily living, and safe mobility support.
  • Ability to follow care plans, observe and report changes in client condition, and maintain accurate records.
  • Effective communication and interpersonal skills, with a commitment to respectful, person-centered care.
