Job Description:

Join our compassionate team as a Home Care Occupational Therapist, dedicated to making a positive impact on the lives of individuals in the comfort of their homes. As a Home Care Occupational Therapist, you will focus on enhancing clients' independence and quality of life through personalized therapy plans.
Key Responsibilities:

1. Conduct in-home assessments to evaluate clients' daily living needs and challenges.
2. Develop and implement customized occupational therapy plans to address clients' specific goals.
3. Provide therapeutic interventions to improve clients' ability to perform daily activities.
4. Collaborate with clients, families, and healthcare professionals to ensure a holistic approach to care.
5. Educate clients and their families on adaptive techniques and equipment for increased independence.
6. Maintain accurate and detailed documentation of assessments, interventions, and progress.
Qualifications:

1. Degree in Occupational Therapy from an accredited program.
2. State licensure as an Occupational Therapist.
3. Strong clinical and communication skills.
4. Ability to adapt therapy plans to diverse client needs.
5. Compassionate and patient-centered approach to care.
6. Valid Driver’s License and/or reliable transportation.

Experience:
