  • Position Summary
    • Under the supervision of Staffing Coordinator, Home Health Aide will assist patients with activities of daily living, personal care, and light housekeeping which is essential to the patient's health care.
  • Education
    • High School Diploma - Preferred
  • Experience
    • 1 Year - Preferred
    • Ability to perform all essential job responsibilities after three (3) month orientation period- Required
  • License & Certification
    • Certified Nursing Assistant (CNA)- Required
    • Basic Life Support (BLS) - Required
  • Core Job Functions
    • Records information accurately and legibly on appropriate forms or in computer devices according to agency policies and procedures. Collaborates with all personnel through patient team conference to identify, clarify and respond to the needs of the patient.             
    • Provides care as indicated on the Aide Assignment Sheet.
    • Maintains reliable transportation and ability to travel as required.
