Job description

Paraprofessional level position responsible for providing residential/in-home services to consumers of varying ages, diagnoses, and needs.

a.     Implement plans of care

b.     Administer medications ordered by physician(s) in accordance with applicable statutes, regulations, policies, and procedures

c.     Maintain medication administration records

d.     Daily documentation (MAR, progress notes, Innovation Waiver Documentation, outcomes, Incident Reports – as needed, etc.)

e.     Implementation of all items on staff checklist in Shift Responsibility Book at each home

f.      Assist consumers with personal care and activities of daily living (ADLs) to include bathing, dressing, feeding, toileting, etc. If a client is unable to complete all ADL’s for any reason (medical, diagnosis, etc) it is the responsibility of staff to ensure that the home is clean and odor free at the end of their shift. 

g.     Work at the group home during the scheduled week if the client is at home sick, out of school, inclement weather, etc.
