We're seeking the right candidate for the position of an FieldCase Manager - RNto serve the Baton Rouge area. In this vital role, you'll ensure that our patients receive the highest level of care while collaborating with a dedicated team, providing exceptional clinical leadership and support.
*Hospice/HH experience preferred*
What You Bring to the Table:
Essential Qualifications:
  • Identify the patient/family's physical, psychosocial, and environmental needs and reassess as needed
  • Provide nursing services in accordance with the POC.
  • Document problems, appropriate goals, interventions, and patient/family response to hospice care.
  • Collaborate with the patient/family attending physician and other members of the IDT in providing patient and family care.
  • Instruct patient/family in self-care techniques when appropriate.
  • Supervise ancillary personnel and delegates responsibilities when required.
  • Complete and submit accurate and relevant clinical notes regarding the patient's condition.
  • Perform supervisory visits to the patient's residence.
The Ideal Candidate:
  • Applicants should possess a current Louisiana RN license in good standing and have at least one year of nursing experience.
  • Home health/hospice experience is preferred, but we will train eager candidates with good clinical experience.
  • Driver's license, insurance and vehicle
  • Utmost compassion and integrity
Other Qualifications
  • Has not been excluded from participating in Medicare and Medicaid programs
  • Maintains eligibility to participate in Medicare and Medicaid programs throughout employment
  • Capable of providing emergency care as needed for any patient.
