Our accredited Diabetes Self-Management Education and Support (DSMES) program-aligned with ADA and ADCES standards-delivers personalized care through live telehealth sessions led by experienced Certified Diabetes Care and Education Specialists (CDCESs) and Nurse Practitioners.
We combine clinical excellence with compassion to help participants build lasting self-management skills and improve health outcomes. As a growing telehealth organization, our team is passionate about expanding access to high-quality diabetes education and supporting people wherever they are on their health journey.

Position Overview
The Certified Diabetes Care and Education Specialist plays a critical role in delivering high quality, evidence based diabetes education and support through a fully remote telehealth model. This clinician partners directly with participants to assess needs, develop individualized care plans, and guide sustainable behavior change. The role blends clinical expertise with coaching, education, and relationship building to improve both clinical outcomes and patient confidence in self management. This position is designed for clinicians who are comfortable working independently, engaging patients virtually, and contributing to a collaborative care environment.
Work Environment Requirements

Job Duties and Responsibilities
  • Conduct comprehensive biopsychosocial and learning needs assessments
  • Develop individualized, participant centered DSMES and MNT care plans
  • Deliver engaging, evidence based diabetes education via telehealth
  • Support behavior change using motivational interviewing and coaching strategies
  • Educate participants on diabetes medications, insulin therapy, CGM devices, and related technologies within scope of practice
  • Interpret CGM and glucose data to guide patient education and goal setting
  • Document all encounters accurately in the EHR within required timelines
  • Monitor participant progress and adjust care plans to improve outcomes
  • Collaborate with referring providers and interdisciplinary care team members
  • Identify needs outside scope and coordinate appropriate referrals
  • Contribute to quality improvement initiatives and outcomes tracking
  • Participate in team meetings, trainings, and program development activities
  • Deliver a professional, compassionate, and culturally responsive patient experience.

Qualifications
  • Certified Diabetes Care and Education Specialist credential required
  • Registered Dietitian
  • Active professional license where applicable based on clinician background and state requirements
  • Minimum 3 to 5 years of experience in diabetes education, DSMES, or chronic disease management
  • Experience delivering care via telehealth preferred

Clinical Knowledge
  • Strong understanding of diabetes pathophysiology and standards of care

  • Experience interpreting continuous glucose monitoring data
