Geriatric Primary Care NP/PA managing chronic conditions of patients Mon-Fri, no weekends/evenings.

Responsibilities:

The Advance Practice Providers we seek are those who have directly related experience in home health, geriatrics, and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patients needs. Manage chronic conditions of assigned patient panel through 30-45 min patient visits. Achieve quality outcomes meeting HEDIS/STAR measures.
