• Conduct thorough patient intakes, focusing on diagnostic and functional assessments, and provide insightful recommendations to primary care providers regarding treatment goals and plans.
• Deliver brief and focused therapeutic interventions aimed at addressing immediate mental health needs and supporting long-term well-being.
• Collaborate with primary care team members to identify patients who require specialty mental health services, ensuring timely referrals and effective follow-up.
• Develop individual relapse prevention strategies and assist clients in achieving stable functioning through ongoing support.
• Monitor the progress of clients and document pertinent information in accordance with established clinical guidelines and documentation standards.
• Provide education and self-management skills that empower patients to take an active role in their recovery journey.
• Participate in team consultations regarding behavioral health services, contributing to program development and evaluation efforts.
• Maintain adherence to the Code of Ethical Conduct for Clinical Professionals and promote a culture of respect, compassion, and integrity within all interactions.
• Carry out additional duties as assigned, supporting the continual improvement of services offered.

The LPCC will work with a diverse range of clients, including adults, children, and families experiencing various mental health challenges. Your expertise will be pivotal in identifying and addressing issues such as anxiety, depression, trauma, and other psychosocial difficulties. • Bilingual proficiency in English and Spanish is preferred, enabling effective communication with a broader patient base.
• Experience working in a telehealth or remote setting, utilizing technology to engage with clients effectively.
• Familiarity with community resources and referral systems that facilitate patient access to additional supportive services.

The annual salary for this position ranges from USD 87,000 to 105,000, commensurate with experience and qualifications.
