Florida Tri County Service Area-Charlotte, Collier and Lee
The Hope Liaison is a performance-based position responsible for increasing access to care primarily in the program they are assigned i.e. Hospice, PACE. Responsible for the identifying and increasing referral sources to meet organizational access to care goals reflecting the community need for the Hope's programs and services. Utilize various resources and educational techniques to enhance referrals. Responsible for identifying opportunities with other health care providers to support enrollment or appropriate admission into the assigned Hope program. Provide community education when requested or approved by direct report. Ensure referral sources choose a Hope Healthcare program for their patients or residents, whether that be Hospice, PACE, Palliative Care, Hope Choices, Hope Bridges etc.
Education and Experience
Minimum of 2 years of proven healthcare sales performance with a degree in marketing, business, public relations or related field or background in healthcare such as nursing, social work, or other patient centered healthcare role. Must have strong communication skills. Previous sales performance training preferred.
Certificates, Licenses, Registrations
Must possess a valid Florida Drivers' License, current auto insurance, and reliable transportation. R Ns, Social Workers must keep all licensing current.
Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs.
Mathematical Skills
Ability to calculate figures and amounts such as averages, proportions, and percentages. Must be able to differentiate mean, mode and average and understand basic concepts of regression analysis.
Reasoning Ability
Ability to listen intently and articulate Hope's position in relation to objections. Ability to communicate, concentrate and document effectively and accurately.
Language Skills
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports and business correspondence. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
*Winkler location only - Bilingual- English/Spanish speaking required.

Position Requirements (not all inclusive)
Excellent presentation skills, ability to effectively establish strong professional relationships with internal and external customers.
Strong written and verbal communication skills. Able to listen intently to referral partners concerns and effectively overcome objections.
Understands market data for the program assigned and able to correlate activities with access goals.
Able to travel within the entire Hope Healthcare service areas and assigned territory
Achieve monthly and quarterly access goals for assigned program.
Successfully execute a weekly, monthly and quarterly strategy to increase market share.
Demonstrate knowledge of statistical data and research tools utilized to assist in the development of marketing and growth initiatives.
Identify areas of opportunity to increase referrals to assigned Hope program.
Educate referral sources about the assigned program's services through planned sales calls and in-services with the appropriate tools and literature. The referral sources may include healthcare practitioners, physician practice offices, facilities such as nursing homes, assisted living facilities and hospitals as well as other healthcare providers i.e. home health agencies, veterans administration, area agency on aging.
Responsible for executing effective sales calls that identify and meet the needs of the referral partners and clearly communicates the features and benefits of the assigned Hope Healthcare program. These include pre and post sales call planning, establishing rapport, effective questioning skills, proposing solutions, handling objections and closing.
Work closely with the assigned clinical leadership to drive growth by educating them on the needs and expectations of the referral partners and patients.
Communicate to clinical leadership on a periodic basis to review the market environment, opportunity for access and the liaison marketing plans in the primary program assigned.
Know the features and benefits of the assigned Hope program services provided. Is able to articulate to healthcare referral partners the competitive advantages, specialty programs and Medicare/Medicaid guidelines.
Document daily sales calls and is proficient in use of the Customer Relationship Manger (CRM) program to maintain current and accurate information regarding referral partners.
Participate in orienting, training and mentoring other liaisons as determined by the Chief of Organizational Excellence
Serve as a consultant to referral partners regarding all programs available from Hope Healthcare.
Participate in continuing education to promote professional growth. Stay updated on medical news, industry changes in the healthcare and senior communities.
Compensation Pay Range:
$53,623.86 - $80,435.79
