Position Overview
We are seeking a skilled and compassionate Family Nurse Practitioner (FNP) to deliver comprehensive primary and preventive healthcare to patients of all ages. Operating within an outpatient physician practice, you will work collaboratively with a supervising physician and an exceptional clinical support staff.
In this role, you will manage a diverse patient panel, focusing on health promotion, disease prevention, and the effective management of acute and chronic conditions. This position emphasizes continuity of care, patient education, and multidisciplinary collaboration to achieve optimal health outcomes.
Key Responsibilities
Comprehensive Care: Assess, diagnose, treat, and manage acute and chronic conditions for a lifespan patient population.
Preventive Medicine: Deliver routine screenings, health risk assessments, and personalized wellness plans.
Patient Advocacy & Education: Educate patients and families on treatment plans, chronic disease management, and preventative health behaviors.
Clinical Collaboration: Partner with supervising physicians, specialists, and multidisciplinary teams to ensure seamless care.
Documentation Excellence: Maintain accurate, timely, and compliant electronic medical record (EMR) charting in accordance with hospital policies and Illinois state regulations.
Qualifications & Requirements
Education & Experience:
Licensure & Certifications:
Required: Current, unrestricted Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN) licensure in the State of Illinois.
Required: National Board Certification as an FNP (FNP-C or FNP-BC) via AANP or ANCC.
Required: Current Basic Life Support (BLS) certification through AHA or American Red Cross.
Required: Active DEA registration and Illinois Controlled Substance License (CSL), or the ability to obtain prior to your start date.

Proven clinical competence in an outpatient primary care setting.
Strong critical thinking, diagnostic reasoning, and independent decision-making capabilities.
Excellent interpersonal and communication skills to build rapport with patients and care teams.
Strong time-management skills with the agility to thrive in a fast-paced clinical environment.
Proficiency with EMR systems and modern healthcare technology.
