Job Title/Position: Clinical Liaison-Luxor Buildings Reports To: Executive Director

JOB DESCRIPTION SUMMARY

The Clinical Liaison develops relationships with healthcare professionals and the community at large, facilitates communication and coordination across the continuum of care, promotes the services provided by One Community Hospice & Palliative Care, and provides hospice education throughout the community.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. Responsible for developing relationships throughout the community to increase awareness of hospice and promote utilization of GCH services.
2. Development of a comprehensive marketing plan for assigned territory that meets budgetary and growth expectations.

3. Develop and maintain relationships with healthcare professionals and/or organizations to facilitate patient access to hospice services.
4. Organizes call schedule for assigned territory and provides weekly report to the Community Relations Director of results oriented goal attainment.
5. Establishes and develops a networking plan to promote hospice services and create concentric circles of influence in the GCH healthcare community.
6. Communicates and collaborates with clinical team members to facilitate admissions to hospice services.
7. Admits new patients and assists with routine RN overflow visits as assigned.
8. Ensures positive referral to admission outcomes for patients, families, and referral services. 9. Establishes regular communication with current referral sources to maintain and develop relationships.
10. Develops and maintains a database for community and professional contacts.
11. Represent the organization in community events, increasing awareness of GCH services. 12. Demonstrates regular and consistent attendance.
13. Ensures the hospice program maintains compliance with all federal and state regulatory and licensure requirements in the promotion of GCH services.
14. Complies with all provisions of the Health Insurance Portability and Accountability Act (HIPAA).
15. Adhere to the provisions of GCH's ethical standards, guidelines, and corporate compliance plan.
16. Models and exemplifies the highest ethical standards in all activities and interactions as a member of the GCH leadership team.
17. Performs other duties as assigned.

POSITION QUALIFICATIONS

1. A four-year degree in healthcare, business, psychosocial, or hospice related field (preferred) with an active MO RN license (required).
2. At least one year of experience in marketing healthcare services and/or products. Previous experience with hospice, home health, acute care, or other post-acute care preferred.

3. Strong communication skills.
4. Ability to generate new community/referral source leads.

5. Internally motivated self-starter able to self-manage at times and lead others through modeling a neighborly consistent approach toward the entire healthcare community.
6. Ability to communicate verbally and in writing to collaborate with the interdisciplinary group and GCH healthcare partners.
7. Demonstrated commitment to the hospice philosophy of care.
8. Possess and provide a valid driver’s license and auto insurance.
