Work Environment

This role requires an onsite presence at our state-of-the-art rehabilitation facility in Sacramento, California. Key Responsibilities
  • Conduct thorough physical therapy evaluations for patients of all ages to assess their functional abilities and limitations.
  • Develop and implement personalized treatment plans tailored to each patient's individual goals, ensuring a compassionate approach in every session.
  • Guide patients and their families in performing therapeutic exercises and daily activities correctly to enhance recovery and mobility.
  • Maintain detailed records of patient progress, addressing any changes in treatment plans or conditions, and communicate effectively with other healthcare professionals.
  • Collaborate with interdisciplinary teams, including physicians, nurses, and social workers, to coordinate holistic care that optimizes patient outcomes.
  • Educate patients and their families about injury prevention techniques and self-care strategies to minimize risks of re-injury.
  • Perform administrative duties such as maintaining compliance with state and national regulations in patient documentation and managing care protocols.
Patient Population / Client Focus

You will work with a diverse patient demographic including individuals recovering from surgery, sports injuries, chronic pain, and neurological conditions. Our patient-first approach emphasizes therapeutic support through individualized care, allowing you to make a meaningful impact on their rehabilitation journey.

Qualifications
  • A Bachelor's Degree in Physical Therapy or a related field from an accredited institution.
  • Current and valid license to practice physical therapy in the state of California.
  • A minimum of two years of professional experience in a clinical setting, ideally within rehabilitation.
Preferred Experience
  • Experience in handling a varied patient workload, effectively creating treatment plans for conditions such as orthopedic injuries, post-surgical recovery, and geriatric concerns.
  • Familiarity with electronic medical record (EMR) systems for documentation and patient management.
  • A commitment to ongoing professional education and adapting to new practices and techniques in physical therapy.
Compensation and Benefits

This position offers a competitive annual salary ranging from $110,000 to $125,000, commensurate with experience and qualifications. In addition, Twin Mountain Physical Therapy provides a comprehensive benefits package that includes:
