Job description

Rafi Health is seeking a full-time Street Medicine Provider to provide care directly to individuals experiencing homelessness throughout Los Angeles. This is a unique opportunity to bring high-quality, compassionate healthcare into the community and serve patients in the environments where they live, including encampments, shelters, and other outdoor settings.

The provider will work collaboratively to deliver primary, acute, preventive, and chronic care, while helping patients navigate ongoing medical and social needs. This role is ideal for a clinician who is passionate about health equity, comfortable working outside of a traditional clinical setting, and motivated to care for some of Los Angeles' most underserved communities.

Responsibilities
  • Provide comprehensive medical care to individuals experiencing homelessness and other underserved populations.

  • Perform evaluations, physical exams, diagnosis, treatment, and medication management.

  • Develop care plans and coordinate referrals, follow-up, and community resources.

  • Build trusting relationships with patients through compassionate, trauma-informed care.

  • Document encounters and support continuity of care across settings.

Qualifications
  • Active, unrestricted California clinical license appropriate to the provider's discipline. 

  • Prior clinical experience required; experience serving underserved or unhoused populations strongly preferred.

  • Experience working with homeless, underserved, or other high-need populations strongly preferred.

  • Street medicine, mobile clinic, shelter, community health, or similar experience is a plus.

  • Strong clinical judgment and communication skills.

  • Ability to work independently and collaboratively in dynamic field environments.

  • Valid California driver's license and reliable transportation.

Work Environment

This is a field-based, outdoor role. Providers should be comfortable delivering care in dynamic and sometimes unpredictable environments. The ideal candidate is passionate about health equity, committed to serving vulnerable communities, and motivated to help improve access to care and positively impact the communities we serve. The role also requires comfort with working outdoors, being assigned anywhere in Los Angeles, and adapting to varying weather and field conditions.
