Job description

JOB SUMMARY:The LVN / LPT provides direct client care including medications and basic medical intervention and education, following standard nursing practices.

MINIMUM QUALIFICATIONS

Education

Graduation from an accredited school of licensed vocational nursing or psychiatric technology required.

Associate's degree, preferred.

Experience

One (1) year service experience working as a nurse in a behavioral health field

One (1) year medication administration experience required.

License or Certification

Valid California LVN or LPT license.

Current CPR certification.


    PHYSICAL REQUIREMENTS

    1. Ability to physically perform Pro-ACT.
    2. Ability to visually and audibly assess participants' behavior and needs.
    3. Ability to provide first aide.
    4. Ability to stand and walk for long periods, up to 3 hours at a time.
    5. Ability to drive within the community in personal vehicle.

    POTENTIAL JOB HAZARDS

    1. Potential for eye and wrist strain from computer screen and minor lacerations from paper cuts.
    2. Repetitive movements - writing and typing.
    3. Interaction with potentially assaultive youth.

    DUTIES AND RESPONSIBILITIES

    Essential Duties

    1. Provides direct nursing care.
    2. Visually and audibly observes, documents in writing and reports orally or in writing the physical or mental condition to the Clinical Director and the Nurse Practitioner or Psychiatrist.
    3. Confirms delivery of all ordered medication from pharmacy on shift worked.
    4. Provides basic medical care, including first aide and medical intervention within scope of licensure as necessary for each individual client.
    5. Documents all services provided in a clear and concise manner within the Electronic Health Record within the timelines and framework established by Stars.
    6. Prepares, presents and coordinates a verbal or written change of shift report.
    7. Ensures compliance with legal, regulatory, and clinical policies and procedures regarding medical care.
