Location: Colorado Springs, Security Widefield, and Fountain, CO
Schedule: Full-Time | Weekly Pay | 8-hour work schedule M-F

***Because of mandatory on-call rotation and possible emergencies, applicants must live within 20 miles of Colorado Springs, CO.***

Are you looking for a meaningful nursing role with steady hours, autonomy, and the opportunity to provide one-on-one care? This Home Health Agency is a Registered Nurse-owned agency and is seeking a Registered Nurse Care Manager to join our growing team!

  • Will train in Home Health only; requires 1 year of nursing experience.

Your Role:

As an RN Care Manager, you will coordinate and provide skilled nursing care in patients’ homes. You will assess patient conditions, develop care plans, and collaborate with the care team to ensure high-quality outcomes. This role will be a combination of medical home care, such as case management of skilled nursing patients, and oversight of non-medical programs.

Responsibilities include:

  • Experience in Adult and Pediatric Care
  • Perform comprehensive assessments (physical, psychosocial, environmental)
  • Maintain detailed clinical documentation in compliance with home health regulations
  • Communicate changes in patient status to physicians and the care team
  • Coordinate care and services with auxiliary staff
  • Take independent and appropriate action in home care situations
  • Continuously work to improve patient outcomes and care quality
  • Coordination of medical and non-medical
  • Proficient in Care Plan Development and collaboration Qualifications
  • In-Home assessment/ Supervisory visits
  • Referral case Coordination
  • Current Registered Nurse license in the state of Colorado & Multi-State
  • CPR certification (current)
  • Strong clinical judgment and communication skills
  • Excellent time management and organizational skills
  • Ability to work independently and manage a flexible schedule
  • Valid driver’s license, auto insurance, and reliable transportation
  • Must meet all federal and state vaccination and health screening requirements (e.g., Influenza and TB)
  • Must be computer literate in Excel and Outlook at a minimum
