Copy of nursing license and Prime Source Verification (PSV). BLS NPDB data sheet - MUST BE SIGNED BY CANDIDATE (attached with the PWS) ** PLEASE MAKE NOTE THIS IS AN SCA POSITION. Suppliers must comply with the SCA rules and regulations according to 2015-4539 r23 Please review the Wage Determination at SAM.gov. ** Legal Authorization to Work in the United States: HCWs performing under this contract must be a U.S. Citizen.
