Job Description

RN Case Manager able to travel to patient homes to conduct home health assessments. The RN here will NOT be conducting assessments of the patient's condition (i.e. no direct patient contact) rather they will be assessing the home health environment based on the patient's medical records and needs to see what, if anything, needs to be setup and then implementing a plan of care.

 Provide in home environmental and physical assessments for high risk members to identify unmet needs and barriers to care and to establish a care management plan of care

Qualifications

Clear and active RN license in state of LA

Bachelor's degree

3+ years case management/health assessment experience

Clear and active driver's license

Hours for this Position:

Full time M-F: daytime hours


Pay Range: $30.00+ (based on experience)
