What you'll do

  • Deliver comprehensive primary care to CareMore ISNP members with complex chronic conditions in SNF, LTC, and institutional settings.
  • Conduct admission evaluations, routine follow-ups, and acute visits.
  • Develop individualized care plans including chronic disease management and advance care planning.
  • Coordinate transitions of care: hospital discharges, readmission prevention, medication reconciliation.
  • Collaborate with nurses, care managers, social workers, specialists, and facility staff.
  • Support STARs/HEDIS quality outcomes and reduce avoidable ER visits and readmissions.
