• Conduct comprehensive assessments to identify the unique needs and strengths of each patient, guiding individualized treatment plans.
• Collaborate with supervisors, direct care staff, and family members to effectively communicate updates concerning client progress.
• Prepare and maintain a tentative schedule for patient appointments, home visits, and therapeutic observations to facilitate consistent progress monitoring.
• Coordinate in-home visits that may include chart reviews, interviews with staff, and thorough data analysis as part of the treatment process.
• Ensure that treatment program plans are implemented effectively while maintaining fidelity to therapeutic modalities.
• Participate actively in Interdisciplinary Team Meetings, sharing insights and best practices to enhance patient care.
• Document clinical progress through monthly and quarterly notes to track advancements in treatment.
• Organize and deliver training sessions to ensure that staff members are fully competent in implementing treatment plans and maintaining accurate documentation.
• Provide follow-up interviews and observations to ensure continuous support for clients, fostering ongoing engagement in their mental health journey.
• Act as a liaison to the broader mental health community, building partnerships that support program development and client goals.

This position primarily serves a diverse clientele, including both adults and youth facing various mental health challenges. You will engage with individuals from different backgrounds, providing culturally sensitive care tailored to their specific needs. The ability to connect with clients and establish rapport is crucial in supporting their mental well-being and fostering positive outcomes.

• A Master’s or Doctoral degree in Counseling, Psychology, or a relevant field is required.
• Active and unrestricted licensure as a Licensed Professional Counselor in the state of Colorado.
• At least two years of counseling experience is mandatory, ideally within a clinical context.
• Strong knowledge of, and adherence to, legal and ethical standards in mental health practice.
• Proficient interpersonal and communication skills to build rapport with a variety of clients.
• A demonstrated ability to work collaboratively within a healthcare team.
• Familiarity with EMR software and efficient documentation practices.

• Specialization in areas such as family therapy, couples therapy, trauma-informed care, or substance misuse is highly desirable.
• Experience in employing evidence-based therapeutic practices to enhance client outcomes.

• Annual salary ranging from USD 80,000 to $105,000, commensurate with experience and caseload.
