As a remote team member, you will work collaboratively with a dedicated group of professionals who are passionate about improving lives. Our full-time position typically follows a schedule from Monday to Friday, 8:00 AM to 4:30 PM, with some flexibility. Given our commitment to our clients' needs, occasional evening or weekend hours may be required. You will ensure continuous and effective care by being available for on-call situations when urgent client assistance is needed.

• Conduct comprehensive assessments of seniors’ psychosocial needs, taking into account emotional, mental, and social factors.
• Collaborate closely with clients and their families to develop individualized care plans that reflect their unique goals and values.
• Provide counseling and emotional support throughout life transitions, erasing the stigma of seeking help.
• Advocate for clients in navigating community resources, ensuring they receive appropriate assistance and services tailored to their needs.
• Work collaboratively with an interdisciplinary team of nurses, caregivers, and physicians to ensure seamless care coordination.
• Regularly visit clients to monitor their well-being, update care plans, and address any changes in their living conditions.
• Facilitate communication among family members, aiming to resolve conflicts and promote better understanding of care objectives.
• Educate clients and their families about relevant community resources, support groups, and available programs to foster their social engagement.
• Maintain accurate client records, ensuring documentation is up-to-date with assessments, care plans, and progress notes.
• Stay informed about current best practices, policies, and ethical standards in social work.

Your primary focus will be on the senior population, many of whom face distinct challenges related to aging, including mental health issues, social isolation, and the need for support during critical life transitions. By utilizing your clinical skills and empathy, you will be instrumental in enhancing their quality of life and helping them navigate the complexities of their situations.

• Master's Degree in Social Work (MSW) from an accredited institution.
• Current and valid licensure as an Independent Clinical Social Worker (LICSW) in the state of Massachusetts.
• A minimum of two years' experience in a social work capacity, ideally within a community or geriatric context.
• Strong understanding of aging-related issues and familiarity with available community resources.
• Excellent interpersonal, assessment, and counseling skills, with a demonstrated ability to build rapport with clients and their families.
• Proficient in electronic health record (EHR) systems and computer applications relevant to case documentation.

• Experience working within community health settings or with older adult populations.
• Knowledge of therapeutic techniques and programs beneficial for seniors.
• Proven track record of effective advocacy for client needs and access to services.
