• Conduct comprehensive assessments of patients' mental health needs, ensuring a thorough understanding of their psychological, social, and emotional conditions.
• Develop and implement personalized treatment plans using evidence-based interventions tailored to the unique circumstances of each patient, particularly focusing on pain management and emotional support during end-of-life transitions.
• Utilize specialized therapy techniques such as cognitive-behavioral therapy (CBT) and solution-focused brief therapy (SFBT) to guide patients and families through their journeys, addressing issues such as grief, anxiety, and depression.
• Maintain accurate and up-to-date clinical records in strict accordance with confidentiality standards and compliance regulations, ensuring that documentation reflects the care provided.
• Collaborate closely with a multidisciplinary treatment team, participating in regular case reviews to ensure a holistic approach to patient care and continuity in treatment.
• Educate patients and their families about mental health resources, coping strategies, and support systems available to them during the hospice process.
• Stay informed about current mental health research and best practices to continuously enhance the quality of care provided.

This role primarily focuses on serving diverse patients facing terminal illnesses, including their families. You will work closely with individuals experiencing complex emotional struggles related to grief, loss, and existential issues. The aim is to help patients attain a sense of peace and understanding, enhancing their quality of life through tailored therapeutic interventions.

• A Master’s degree in Social Work from an accredited institution is essential.
• Possession of a valid LCSW license in the state of Illinois.
• At least three years of clinical experience specializing in psychotherapy, particularly with end-of-life and hospice populations.
• Proficiency in evidence-based psychotherapy modalities, particularly solutions-focused interventions.
• Strong communication and problem-solving skills, with a proven ability to manage complex patient cases in a collaborative environment.

• Experience working in a hospice or palliative care environment is highly preferred.
• Familiarity with case management software and electronic health records (EHR) systems to maintain efficient clinical documentation.
• Active engagement in community outreach programs to facilitate holistic care approaches.

The position offers a competitive salary range of USD 85,000 to 105,000 annually, complemented by a sign-on bonus. In addition to financial compensation, we offer:
