Job Description:
Essential Functions
โ€ข Provides in person risk assessments.
โ€ข Develops, implements, and evaluates individualized care plans in collaboration with patients, families, and the healthcare team to address identified needs and goals.
โ€ข Provides patient safety planning for patients at risk of suicide.
โ€ข Makes outside referral and phone follow-up calls.
โ€ข Determines appropriate level of care.
โ€ข Facilitates the admission process.
โ€ข Maintains communication with referral sources, families, patients and unit staff.
โ€ข Collaborates with interdisciplinary teams to achieve quality, safety, and patient-centered care goals, participating in process improvement initiatives.
โ€ข Engages in ongoing professional development, maintains required licensure and certifications, and completes mandatory competencies and training.
โ€ข Performs other duties as assigned.
Education Requirement
Associate's degree in nursing
Bachelor's degree in nursing (BSN) is preferred
Experience Requirement
1-2 years of experience in nursing, or a relevant area
An equivalent combination of education and experience may be considered.
All experience must be paid and in the same related field.
Part-time and PRN experience will be prorated based on hours worked per week.
Volunteer work and internships for academic credit are not counted.
Certifications & Licensures
Current and active Registered Nurse (RN) license, Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS).
PMH-BC certification is preferred for this position.
Based on assigned area: Non-Violent Crisis Intervention (CPI), NIH Stroke Scale Certified, Oncology Nurse Certified (AOCN, BMTCN, CPHON, CPON, CBCN, OCN), Neonatal Resuscitation Program (NRP), Pediatric Advanced Life Support (PALS), TNCC or BTLS, Mental Health Nurse Certified
