Position Overview

The Together & Strong Counseling Center is actively seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our hospice care team. This full-time, remote position will play a vital role in providing comprehensive therapeutic support to clients at various stages of mental health challenges, ensuring that they and their families receive the exceptional care needed during sensitive times. The salary for this position ranges from USD 70,000 to 95,000 annually, commensurate with experience and qualifications.

Key Responsibilities
  • Conduct thorough assessments and develop personalized treatment plans tailored to individuals, couples, and families, addressing a variety of mental health disorders, including anxiety, depression, trauma, and grief.
  • Utilize evidence-based therapeutic techniques such as Cognitive Behavioral Therapy (CBT) and dialectical behavior therapy (DBT) to implement effective interventions and support client progress.
  • Regularly monitor client progress and adjust treatment plans as necessary, conducting timely reassessments and making referrals to other healthcare providers when appropriate.
  • Provide crisis intervention services, facilitating immediate and effective support during acute emotional distress or mental health crises.

  • Collaborate with a multidisciplinary team of healthcare professionals, providing consultation and guidance to support integrated care for clients.

Patient Population / Client Focus

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited institution is required.
  • Active Licensure as a Clinical Social Worker (LCSW) in the state of Pennsylvania.
