The Together & Strong Counseling Center is actively seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our hospice care team. • Conduct thorough assessments and develop personalized treatment plans tailored to individuals, couples, and families, addressing a variety of mental health disorders, including anxiety, depression, trauma, and grief.
• Utilize evidence-based therapeutic techniques such as Cognitive Behavioral Therapy (CBT) and dialectical behavior therapy (DBT) to implement effective interventions and support client progress.
• Regularly monitor client progress and adjust treatment plans as necessary, conducting timely reassessments and making referrals to other healthcare providers when appropriate.
• Provide crisis intervention services, facilitating immediate and effective support during acute emotional distress or mental health crises.
• Build strong, trusting relationships with clients and their families, maintaining open lines of communication to effectively address concerns and promote engagement in the therapeutic process.
• Collaborate with a multidisciplinary team of healthcare professionals, providing consultation and guidance to support integrated care for clients.
• Participate in regular clinical meetings, quality assurance activities, and professional development workshops to enhance clinical skills and knowledge.
• Maintain meticulous documentation of client services in compliance with regulatory and organizational standards, ensuring confidentiality and accuracy in record-keeping.

In this role, you will primarily work with clients facing severe mental health challenges, many of whom are navigating the complex emotional landscapes associated with hospice care. This includes providing support to individuals coping with terminal diagnoses, grief, and loss, as well as offering guidance to families adjusting to challenging life transitions. Your role will be crucial in facilitating open dialogue, healing, and closure for those experiencing the significant changes that accompany end-of-life circumstances.

• A Master’s degree in Social Work from an accredited institution is required.
• Active Licensure as a Clinical Social Worker (LCSW) in the state of Pennsylvania.
• Demonstrated knowledge and application of psychological theories and therapeutic modalities tailored for diverse populations.
• Exceptional interpersonal and communication skills, with the ability to connect deeply with clients and their families.
• A strong ability to manage multiple cases effectively while maintaining a high standard of care and ethical practice.
