Job description

We're partnering with a digital health company that supports patients at risk for, or living with, chronic kidney disease (CKD), and we're looking for Registered Nurse (RN) Care Managers to join their team. This is a fully remote, full-time opportunity for nurses passionate about coaching, patient education, and supporting behavior change in chronic care.What You'll Do:Connect with patients via phone, video, and messaging to support their health goalsUse motivational coaching to guide behavior change and assess health needsDevelop individualized care plans (CKD experience preferred)Coordinate care and collaborate with providers as neededWhat We're Looking For:Licensure: RN license in good standing; an additional multi-state compact license is preferredExperience: 4+ years of nursing experienceAdditional Training: CDE or CDCES, and/or training in health coaching or motivational interviewing strongly preferredBilingual (Spanish) is highly preferredTravel: Ability and willingness to travel 1-3 times per year for corporate gatherings, conferences, or health fairs, including overnight staysLocation: Based in the Pacific, Alaska, Arizona, or Mountain time zone.
