• Conduct comprehensive psychosocial assessments to understand clients’ mental health, strengths, and available resources.
• Facilitate individual, family, and group therapy sessions utilizing various evidence-based treatment modalities, such as cognitive-behavioral therapy and solution-focused therapy.
• Develop, implement, and evaluate client-centered treatment plans tailored to meet the unique needs of each individual.
• Provide crisis intervention, risk assessment, and safety planning to help stabilize clients in high-stress situations.
• Document therapy sessions and treatment plans accurately and in a timely manner, in line with ethical and legal standards.
• Facilitate group therapy sessions and psychoeducational programming to empower clients and their families through shared experiences.
• Work collaboratively with a multidisciplinary team to ensure that clients receive comprehensive and coordinated care.

In this role, you will primarily work with individuals facing end-of-life challenges and their families, who often have complex emotional and psychological needs. • Master’s degree in Social Work from an accredited institution.
• Active and current state licensure as a Licensed Clinical Social Worker (LCSW).

• Proficiency in evidence-based therapeutic practices and a strong understanding of treatment modalities applicable to diverse diagnoses.

• Experience in hospice, palliative, or end-of-life care.
• Demonstrated ability to work within a multidisciplinary team and collaborate effectively with various health professionals.
• Familiarity with documentation and electronic health record systems relevant to mental health practices.
