This position is fully remote, allowing for greater flexibility while ensuring that our clients receive uninterrupted support. As a member of our hospice care team, you will engage with clients via telehealth platforms, working from a HIPAA-compliant home office equipped with high-speed internet. • Deliver personalized individual, group, and family therapy sessions tailored to the unique needs of hospice patients and their families.
• Conduct thorough psychosocial assessments to understand patients’ emotional and environmental challenges, thereby facilitating the development of effective treatment plans.
• Utilize evidence-based therapeutic techniques such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and trauma-informed care to address complex issues.
• Provide crisis intervention and case management, ensuring that patients receive timely support and resources when they need it most.
• Maintain accurate, confidential documentation within our electronic health record (EHR) system, contributing to a seamless flow of information within the multidisciplinary team.
• Collaborate closely with psychiatrists, psychologists, nurses, and other healthcare professionals to coordinate care and advocate for patient needs.
• Participate in regular case reviews and clinical meetings, contributing insights and engaging in team discussions to enhance patient care strategies.
• Ensure compliance with HIPAA regulations and state licensing requirements, safeguarding the confidentiality and integrity of patient information.

The ideal candidate will understand the sensitivities associated with providing care to hospice patients—individuals nearing the end of their lives and their families facing emotional turmoil. You will play a crucial role in not only guiding patients through their grief and concerns but also empowering families to cope with their loved ones’ transitions with dignity and support.

• An active LCSW license in the state of California, in good standing.
• A Master’s Degree in Social Work (MSW) from an accredited program.
• A minimum of 2 years of post-licensure clinical experience, demonstrating expertise in mental health intervention.
• Strong proficiency in evidence-based therapeutic approaches, including CBT, DBT, Acceptance and Commitment Therapy (ACT), and trauma-focused therapy.
• Exceptional communication, assessment, and counseling skills to effectively engage with patients and families in a virtual setting.
• Experience with electronic health record (EHR) systems and a thorough understanding of clinical documentation practices.

• Experience delivering telehealth services and familiarity with the specific challenges and advantages they present in meeting client needs.
• Previous experience in hospice or palliative care settings, with a focus on patient and family counseling.

Opti Mind Behavioral Health offers a competitive salary range of $100,000 - $130,000 per year, commensurate with experience.
