As a member of our hospice care team, you will engage with clients via telehealth platforms, working from a HIPAA-compliant home office equipped with high-speed internet. Key Responsibilities
  • Deliver personalized individual, group, and family therapy sessions tailored to the unique needs of hospice patients and their families.
  • Conduct thorough psychosocial assessments to understand patientsโ€™ emotional and environmental challenges, thereby facilitating the development of effective treatment plans.
  • Utilize evidence-based therapeutic techniques such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and trauma-informed care to address complex issues.
  • Provide crisis intervention and case management, ensuring that patients receive timely support and resources when they need it most.
  • Maintain accurate, confidential documentation within our electronic health record (EHR) system, contributing to a seamless flow of information within the multidisciplinary team.
  • Collaborate closely with psychiatrists, psychologists, nurses, and other healthcare professionals to coordinate care and advocate for patient needs.
  • Participate in regular case reviews and clinical meetings, contributing insights and engaging in team discussions to enhance patient care strategies.
  • Ensure compliance with HIPAA regulations and state licensing requirements, safeguarding the confidentiality and integrity of patient information.
Patient Population / Client Focus

The ideal candidate will understand the sensitivities associated with providing care to hospice patientsโ€”individuals nearing the end of their lives and their families facing emotional turmoil. You will play a crucial role in not only guiding patients through their grief and concerns but also empowering families to cope with their loved onesโ€™ transitions with dignity and support.

Qualifications
  • An active LCSW license in the state of California, in good standing.
  • A Masterโ€™s Degree in Social Work (MSW) from an accredited program.
  • A minimum of 2 years of post-licensure clinical experience, demonstrating expertise in mental health intervention.
  • Strong proficiency in evidence-based therapeutic approaches, including CBT, DBT, Acceptance and Commitment Therapy (ACT), and trauma-focused therapy.
  • Exceptional communication, assessment, and counseling skills to effectively engage with patients and families in a virtual setting.
  • Experience with electronic health record (EHR) systems and a thorough understanding of clinical documentation practices.
Preferred Experience
  • Experience delivering telehealth services and familiarity with the specific challenges and advantages they present in meeting client needs.
