Work Environment

This role is situated in a remote work setting, allowing you to manage your own schedule while being part of a supportive and dynamic team. Our hospice center serves as the home base from which we deliver crucial early intervention services, ensuring that children and their families receive the quality care necessary for healthy development. By utilizing modern communication tools, you will maintain consistent contact with team members and families to foster collaboration and shared goals.

Key Responsibilities

• Conduct comprehensive evaluations and assessments to determine the developmental needs of young children.
• Engage actively in the Individualized Family Service Plan (IFSP) development and implementation process, tailoring strategies to meet each child's unique needs.
• Monitor and evaluate outcomes, collaborating closely with a multidisciplinary team to ensure that children receive effective and evidence-based interventions.
• Provide essential support to families, equipping them with practical strategies and resources to enhance their child's development.
• Maintain compliance with all federal and state regulations, adhering to best practices in social work and early intervention.
• Foster rapport and trusting relationships with children, caregivers, and team members to create a nurturing and supportive learning environment.
• Utilize Natural Learning Environment Practices to implement the IFSP effectively, adjusting service frequency according to the child's developmental status and family priorities.
• Deliver information and training to families, empowering them with the skills necessary to support their child's growth.
• Implement effective coaching models and natural environment teaching methods seamlessly in your practice.
• Uphold high standards of health and safety for all staff and individuals served.

Patient Population / Client Focus

Your primary focus will be on infants and toddlers with developmental challenges or delays, specifically during the critical early years of life. By addressing physical, cognitive, social-emotional, and communication skills, you will help these children unlock their potential while providing essential support to their families in navigating the early intervention process. You will engage with a diverse client population, necessitating cultural sensitivity and an adaptable approach to meet varied needs.

Qualifications

• Master's Degree or Doctorate in Social Work, with a valid license issued by the Department of Public Health.
• Current LCSW designation is a requirement for this role.
• A valid driver’s license is necessary to travel as required for home visits or meetings.
• Exceptional ability to engage with families and children utilizing various learning styles and approaches tailored to their individual treatment plans.
• A minimum of one year of experience working in early intervention settings, directly supporting children and families.
• Demonstrated commitment to utilizing evidence-based practices within a Natural Learning Environment framework.

Compensation and Benefits
