As a Licensed Clinical Social Worker, you will:

• Conduct comprehensive bio-psychosocial evaluations that identify psychosocial problems impacting patients and their families.
• Develop, implement, and evaluate individualized treatment plans while employing a range of evidence-based therapeutic modalities, including both individual and group therapies.
• Document social work activities and patient interactions in adherence to departmental standards, ensuring accurate and timely records in patient medical files.
• Consult with medical staff about the psychosocial needs and backgrounds of patients to streamline treatment and improve care coordination.
• Provide education to patients and families regarding treatment recommendations and psychosocial adjustments needed for optimal functioning.
• Advocate for patient and family needs, connecting them with appropriate community resources that enhance their support systems.
• Engage in ongoing professional development to stay current with best practices, attending relevant training sessions focused on hospice care and mental health.

Your primary focus will be on patients and families receiving hospice care, navigating the complex and emotional challenges associated with serious illnesses. You will provide crucial support during a pivotal time, addressing not only mental health needs but also offering guidance on coping mechanisms, family dynamics, and community resources. Your understanding of diverse cultural backgrounds will enhance your ability to serve our varied patient population effectively.

To be considered for this role, candidates must possess:

• A Master’s Degree in Social Work from an accredited institution.
• A current Licensed Clinical Social Worker (LCSW) license issued by the State Board of Social Work Licensure.
• A solid foundation in social work principles and a deep understanding of the aging process, human development, and cultural considerations impacting mental health care.

The ideal candidate will have:

• Six months to one year of clinical experience working in a health or mental health setting, ideally within hospice or palliative care.
• Demonstrated ability to address the unique needs of patients facing end-of-life care, with sensitivity to the emotional and psychological complexities involved.
• Experience collaborating with multidisciplinary teams to deliver coordinated care.
