Job Summary
Port Rehabilitation and Healthcare Center is seeking dedicated and compassionate Registered Nurses (RNs) to join our team of healthcare professionals.
As an RN at Port Rehabilitation and Healthcare Center, you will provide direct nursing care to our residents while overseeing and supporting the day-to-day nursing activities performed by licensed practical nurses (LPNs) and certified nursing assistants (CNAs). You will work closely with residents, families, physicians, and the interdisciplinary care team to ensure high-quality, individualized care.
Responsibilities
  • Provide direct nursing care to residents, including assessment, care planning, implementation, and evaluation
  • Supervise and delegate appropriate nursing tasks to LPNs and CNAs
  • Monitor residents' conditions and communicate changes to physicians and other members of the healthcare team
  • Ensure nursing care is provided in accordance with current federal, state, and local regulations, guidelines, and facility policies
  • Develop and maintain positive relationships with residents, families, and healthcare team members
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans
  • Maintain accurate and timely clinical documentation
  • Participate in quality improvement initiatives and contribute to a culture of excellence in resident care
  • Promote a safe, respectful, and supportive environment for residents
Requirements
  • Current RN license in the State of Massachusetts
  • Strong communication and interpersonal skills
  • Strong critical thinking and problem-solving abilities
  • Ability to work effectively in a fast-paced healthcare environment
  • Ability to work collaboratively as part of an interdisciplinary team
  • Ability to maintain confidentiality and appropriately handle sensitive resident information
  • Skilled nursing, long-term care, or rehabilitation experience is a plus
