About Us

Personalized Healthcare of Arizona is a membership-based primary care practice dedicated to delivering highly personalized, relationship-centered healthcare. Our team is committed to providing exceptional patient experiences through compassionate care, proactive communication, and a focus on long-term wellness.

Position Overview

The Clinic Registered Nurse provides responsive, relationship-based care within a membership primary care practice. A crucial function of this role is reviewing and identifying abnormal laboratory and diagnostic testing to the physician; communicating significant findings clearly and efficiently. The RN also works alongside other clinical staff as part of a dedicated triage team, helping patients receive timely guidance, follow up, and personalized support.

Essential Responsibilities

In accordance with the AZSBN RN scope of practice:

  • Review incoming imaging and laboratory results; communicating significant changes, abnormal trends, and critical values in a timely, clear, and concise manner to the physician.
  • Coordinates care by documenting physician notification, patient instructions, and communicates ascribed plans and education to patients.
  • Partners with other clinical team members in the triage of clinic patients by secure texting platform, telephone, or face to face interactions.
  • Assesses and documents patient concerns, gathers focused clinical information, symptom urgency, and escalates concerns to the physician, as appropriate.
  • Promptly responds to patient inquiries via telephone or secure texting with compassion, reassurance, education, and care coordination, when needed.
  • Communicates provider plan of care, orders, and guidance to patients, as directed by the physician.
  • Evaluates patient response to plan of care and adjusts, as appropriate.
  • Uses approved protocols or standing orders, administers injections, vaccines, medications and treatments, performs focused assessments such as lung auscultation, functional lung testing, bladder scans, ear lavage, or other clinic specific procedures.
  • Maintains accurate, timely, and confidential documentation of lab and diagnostic reviews, assessments, triage decisions, patient communications, treatments, and follow up.

Position Focus

The successful RN will combine strong clinical judgment with concise provider communication, compassionate patient support, and efficient care coordination. This role requires an approachable nurse who can identify and communicate important findings, help patients feel heard, provide timely access to the care team, and build lasting relationships with patients while collaborating closely with the care team.

Requirements

Applicants should have a current state RN license and have graduated from an accredited nursing school. Additional requirements include:

  • Certifications: AHA Basic Life Support (BLS) required or obtained within 30 days of hire.
  • Licenses: Must have a current and unencumbered Arizona or compact multi-state RN license.
  • Education: Associate's degree required. BSN preferred.

What We Offer

  • Competitive compensation and benefits package including medical, dental, vision, 401 (k), and Paid Time Off.
  • Comprehensive onboarding and ongoing mentorship.
  • Opportunities for professional development and advancement within a collaborative, vision-driven practice.
  • A supportive environment that values initiative, teamwork, and innovation in healthcare.
