Job description

About the Role
Healthonomic Primary Care is hiring a full-time Nurse Practitioner for our Orland Park and Palos Heights offices. You'll care for an established adult panel, with an emphasis on chronic disease management and preventive care, working closely with our physician team.
Responsibilities
  • Provide primary care to adult patients, including chronic disease management
  • Perform Medicare Annual Wellness Visits and preventive care visits
  • Support our chronic care management program, including patient care plans and follow-up
  • Order and interpret labs and imaging; manage medications and referrals
  • Document care in Epic and support quality and preventive care initiatives
Requerments 
  • Active Illinois APRN/Nurse Practitioner license in good standing
  • FNP or AGNP national certification (ANCC or AANP)
  • Active DEA and Illinois controlled-substance registration, or eligible to obtain before start
  • Current BLS certification
  • 2+ years of nurse practitioner experience in primary care, family practice, or internal medicine
  • Chronic disease management experience with adult patients
  • Comfortable documenting in an EHR; Epic experience preferred
  • Able to work across two offices weekly — Orland Park Monday–Tuesday, Palos Heights Wednesday–Friday
Preferred: Chronic Care Management (CCM) or care-coordination experience, Medicare Annual Wellness Visits, quality/HEDIS familiarity.
