At Community Health Center of the North Country, we believe high-quality healthcare starts with a strong, collaborative team. Our Tactical Registered Nurses play a key role in keeping our health centers running smoothly by providing expert clinical support, coordinating patient care, and helping the clinical team respond effectively to changing patient and provider needs.
Rather than being assigned to a single provider, the Tactical RN works across the health center to support patient flow, manage higher-acuity clinical needs, provide triage, and serve as a clinical resource to the entire team.
What You'll Do:
1. Responsible for supporting clinical operations, patient flow, and the clinical team.
  • Monitor provider schedules and clinical workflows and respond to coverage needs, delays, and other barriers to efficient patient care.
  • Serve as a clinical resource to providers, LPNs, MAs, and other members of the care team, providing informal clinical oversight and direction.
  • Provide clinical coverage as needed, including rooming specialty patients or supporting the team when regularly assigned clinical staff are unavailable.

2. Responsible for providing clinical care at the top of the RN scope of practice.
  • Utilize clinical judgment and comprehensive assessment skills to address higher-acuity and complex patient needs, including triage, interventions, and collaboration with providers.
  • Administer medications and immunizations and perform nursing interventions as ordered.
  • Provide patient education, care coordination, and follow-up for complex medical needs.

3. Responsible for managing clinical triage and telephone-based patient care.
  • Manage triage calls and telephone encounters for walk-in and scheduled patients, as well as those routed from the Call Center.
  • Assess symptoms, provide appropriate nursing guidance, communicate provider recommendations and results, and facilitate escalation when needed.
  • Accurately document clinical assessments, communications, and follow-up in the electronic health record.

4. Responsible for coordinating patient care and supporting providers.
  • Collaborate with providers and clinical staff to address same-day needs, schedule changes, and other clinical issues.
  • Assist providers with examinations, procedures, and treatment interventions and coordinate follow-up care, referrals, and testing as needed.
  • Manage prior authorization requests and appeals requiring clinical review and collaborate with providers, payers, pharmacies, and others to facilitate timely access to care.

5. Responsible for promoting quality, compliance, and professional nursing practice.
  • Maintain accurate documentation and comply with clinical, regulatory, infection control, medication administration, and organizational requirements.
  • Participate in team meetings, quality improvement activities, and other initiatives supporting safe, effective patient care.
  • Maintain professional competency through continuing education, training, and current RN licensure and BLS certification.

6. Performs other duties as assigned by supervisor.

  • Graduate of an accredited Registered Nurse program required, with one year of clinic-based work experience preferred.
  • Current and valid Registered Nurse license in the State of New York.
  • Current BLS certification (may be obtained after hire).
  • Strong clinical assessment, critical thinking, and decision-making skills.
  • Excellent communication, organization, and prioritization skills.
  • Ability to work independently, exercise sound clinical judgment, and collaborate effectively with a multidisciplinary team.
  • High level of professionalism, confidentiality, accountability, and flexibility.
  • Strong computer skills and experience with electronic medical records.
  • Previous primary care, ambulatory care, triage, or care coordination experience preferred.
