Job description

Position Summary

Championed Minds Behavioral Health, LLC is seeking a compassionate, organized, and motivated Full-Time Case Manager to provide community-based behavioral health services to children, adolescents, and families. The Case Manager will coordinate care, assist clients in accessing needed resources, advocate for clients, and collaborate with treatment teams to support positive outcomes.

The ideal candidate is passionate about helping youth and families overcome barriers, improve functioning, and achieve treatment goals while maintaining compliance with Ohio Medicaid and agency requirements.

Essential Job Duties

Care Coordination

  • Conduct comprehensive assessments of client needs and strengths.
  • Develop and implement individualized service plans in collaboration with clients, families, and treatment teams.
  • Coordinate services with schools, medical providers, social service agencies, and community organizations.
  • Monitor client progress and update service plans as needed.
  • Assist families in navigating behavioral health, educational, and community support systems.

Client Support & Advocacy

  • Provide Community Psychiatric Supportive Treatment (CPST) services as appropriate.
  • Link clients and families to community resources including housing, food assistance, transportation, healthcare, educational supports, and employment resources.
  • Advocate for clients to ensure access to necessary services and supports.
  • Assist families in crisis prevention and problem-solving.

Documentation & Compliance

  • Complete all required documentation within agency timelines.
  • Maintain accurate and timely progress notes, service plans, assessments, and other clinical records.
  • Ensure compliance with Ohio Medicaid, OhioMHAS, CARF standards, and agency policies.
  • Participate in audits, quality assurance reviews, and corrective action processes when necessary.

Collaboration

  • Participate in treatment team meetings and case consultations.
  • Maintain communication with referral sources and community partners.
  • Collaborate with therapists, behavioral health specialists, school personnel, and other providers to ensure continuity of care.

Community-Based Services

  • Provide services in homes, schools, community settings, and agency locations.
  • Maintain a flexible schedule to meet client needs.
  • Travel throughout designated service areas.

Minimum Qualifications

Required

  • Bachelor's Degree in Social Work, Psychology, Human Services, Counseling, Criminal Justice, Education, related field, or 3+ years experience. 
  • Valid driver's license and reliable transportation.
  • Ability to pass background checks and meet Medicaid provider requirements.
  • Strong organizational, communication, and documentation skills.
  • Experience working with children, adolescents, and families.

Preferred

  • LSW or other behavioral health credential.
  • Experience with Ohio Medicaid behavioral health services.
  • Experience providing CPST, TBS, or case management services.
  • Knowledge of trauma-informed care and behavioral health interventions.

Required Competencies

  • Ability to work independently and as part of a multidisciplinary team.
  • Crisis intervention and de-escalation skills.
  • Cultural competency and sensitivity.
  • Time management and organizational skills.
  • Proficiency with electronic health records and Microsoft Office applications.

Physical Requirements

  • Ability to travel throughout the community.
  • Ability to sit, stand, walk, and drive for extended periods.
  • Ability to lift up to 20 pounds occasionally.

Schedule Expectations

  • Full-Time Position (40 hours per week)
  • Some evening hours may be required to meet client and family needs.
  • Participation in staff meetings, trainings, supervision, and professional development activities is required.
