SynergenX-Low-T, a leader in hormone replacement therapy and targeted wellness, is seeking a Full-Time 4 day Float Mid-Level Provider (NP or PA). This is an exciting opportunity for providers who value patient-centered care, work-life balance, and a collaborative clinic environment.Key Responsibilities
  • Provide hormone replacement, weight management, and targeted wellness care
  • Conduct patient assessments, create individualized treatment plans, and follow-up care
  • Educate patients on wellness, treatment protocols, and lifestyle optimization
  • Collaborate with Center Director, Medical Director, and clinic staff
  • Maintain accurate documentation and compliance with clinic protocols
Qualifications
  • Active Indiana NP or PA license in good standing
  • Current DEA license or ability to obtain
  • Strong communication and clinical decision-making skills
Schedule
  • 32 clinical hours per week
  • Clinical shifts: Tuesday, Thursday, Friday, and Saturday's
  • Clinic hours: Mon 8-5 | Tues 8-7 | Wed 8-1 | Thurs 7-5 | Fri 8-5 | Sat 8-12
