The primary purpose of a Population Health Coordinator is to improve the health outcomes of a population group by monitoring and identifying individual patient needs within the group
Qualifications
1-2 years in population health management or strong clinical background in nursing preferred
Work experience in community outreach, data collection and analysis, Performance Improvement and Quality Assurance preferred
BLS Required
Healthcare software system knowledge and report building skills preferred
Experience with quality improvement programs, integration and informatics related to planning and population health management. Knowledge of patient centered medical homes, Meaningful Use, MACRA/MIPS, and EMR based reporting. Skill in interacting and communicating with patients, physicians and care coordinators
