This is a full-time RN opportunity, allowing you to truly connect with patients and their families.

As a Home Care RN, you will deliver highquality, patientcentered care to individuals requiring nursing services outside of the traditional clinical setting. Working closely with a multidisciplinary team-including therapists, social workers, physicians, and other healthcare professionals-you'll ensure each client receives a comprehensive, individualized plan of care designed to support their health, safety, and overall wellbeing.

You will provide clinical care in alignment with physician orders and under the guidance and supervision of a Clinical Supervisor. Additionally, you will build meaningful relationships with clients and families, offering not only skilled nursing services but also reassurance, education, and emotional support throughout their care journey.

Responsibilities

Skilled Nursing Care & Patient Assessment

    • Perform comprehensive patient assessments during home visits, evaluating physical, emotional, and environmental needs.
    • Deliver skilled nursing care in accordance with physician orders and individualized care plans.
    • Monitor patient progress regularly to ensure goals are met and adjust care interventions as needed.

Teaching, Supervision & Counseling

      • Educate clients and caregivers on disease management, medications, treatments, and selfcare techniques.
      • Provide ongoing supervision and support to patients and their families, helping them gain confidence in managing health needs at home.
      • Offer counseling and guidance to promote patient independence, safety, and wellbeing.

Interdisciplinary Collaboration

    • Work closely with an interdisciplinary team-including physicians, therapists, social workers, and other healthcare professionals-to coordinate and optimize patient care.
    • Communicate changes in patient condition or care needs promptly to ensure seamless teamwork and continuity of care.

Clinical DecisionMaking & Problem Solving

    • Apply strong critical thinking skills to identify changes in patient status, resolve issues proactively, and implement appropriate nursing interventions.
    • Develop safe and effective solutions to support patient outcomes and address emerging challenges in the home setting.

Documentation & Compliance

 

    • Complete timely and accurate OASIS documentation in accordance with regulatory, organizational, and clinical standards.
    • Maintain clear and thorough records of assessments, interventions, patient responses, and communications.

Specialized Treatment & Procedure Management

    • Provide care for a wide range of physicianordered treatments, including:
      • PleurX drain management
      • Fingerstick coagulation checks
      • Wound care and wound vac therapy
      • IV administration
      • Phlebotomy
      • Tracheostomy care
      • PEG tube management
      • Ostomy care
      • Telehealth monitoring
    • Ensure all procedures are performed using proper technique, infection control practices, and patient safety standards.
Qualifications
  • Minimum of one (1) years recent experience as a professional nurse in Home Health.
  • Must complete Point of Care Testing training as part of Department Orientation.
  • Valid DL With No More Than 3 Moving Violations
Credentials
  • RN - Registered Nurse
  • BLS - Basic Life Saving certification (American Heart Association)
Education
  • Graduate of an accredited nursing program
