Work Environment

This full-time position offers a flexible remote work setting, enabling you to focus on client care from the comfort of your home while maintaining a work-life balance. There are no evening or weekend shifts, allowing you to enjoy regular hours from Monday to Friday.

Key Responsibilities
  • Conduct initial client assessments to understand their goals, preferences, and available resources for creating customized rehabilitation strategies.
  • Design and implement individualized rehabilitation plans that consider clients' educational backgrounds, skill sets, physical capabilities, and vocational ambitions, with the flexibility to adapt these plans as necessary.
  • Collaborate closely with our multidisciplinary team of healthcare professionals to devise and deliver rehabilitation programs tailored to the unique needs of each client.
  • Maintain comprehensive case files that document each client's personal information, eligibility criteria, insurance details, services rendered, and communication notes, ensuring accuracy and up-to-date records.
  • Regularly monitor client progress through detailed documentation and assessments, making necessary adjustments to strategies for optimal outcomes.
  • Engage with clients throughout their participation in job training, placement programs, and on-site activities to quickly address any challenges that arise.
  • Facilitate engagement with clients' family members and support systems to effectively address their mental health needs collaboratively.
  • Conduct timely assessments of client needs and prepare comprehensive reports to inform treatment pathways.
  • Provide directional support to relevant resources and referrals as needed to enhance client care.
Patient Population / Client Focus

Our clients encompass a diverse array of individuals facing mental health challenges, including those dealing with mood disorders and various psychosocial difficulties. You will be instrumental in supporting clients from varied backgrounds, ensuring that each person receives individualized attention based on their unique circumstances and goals.

Qualifications
  • A masterโ€™s degree in counseling, social work, psychology, or a related field.
  • Current licensure as a Licensed Professional Counselor (LPC).
  • Experience in mental health treatment across both inpatient and outpatient settings, focusing on mood disorders and mental health challenges.
  • A collaborative spirit and passion for facilitating positive change in clientsโ€™ lives.
  • Strong clinical competencies in motivational interviewing, cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and meticulous documentation practices.
Preferred Experience
  • Experience working within a community health setting, particularly with diverse populations.
  • Familiarity with integrated care models and rehabilitation programs.
  • Strong ability to work independently as well as collaboratively within a team environment.
